Hours and Payment Forms

All forms should auto-calculate the total hours billed from the # of hours column(s). As well as the total being paid from the total hours billed and hourly rate. Please contact cynthia.chao@vch.ca if you run into any issues and let them know what software you were using when the issue occurred.

For fillable PDFs: Please save the form relevant to your work (groups or 1-1), fill it out using Adobe Reader, save the completed form, and submit to your supervisor/coordinator for payment.

Note: These sheets all assume payment is coming from the PSW program Central Office cost centre. If your payment comes from another cost centre, you can use one of the fillable PDFs below where the cost centre has been left blank.
