The Faculty of Pharmaceutical Sciences has opened the Pharmacists Clinic, a patient care clinic on the Vancouver campus of UBC. The clinic’s focus is on helping patients understand their mediations and providing consultation services to patients and caregivers to optimize drug therapy outcomes. Pharmacists at the clinic work together with patients, their current pharmacies and their physicians to ensure continuity of care in a respectful, collaborative manner. Patients who may benefit from this service have complex drug regimens, chronic diseases, or questions about their medications. The Pharmacists Clinic is a licensed pharmacy however it does not offer prescription-filling services.
As a model site for practice innovation, teaching and research, the clinic is funded by the Faculty and provides services free of charge to patients. Appointments are available in person at the UBC Vancouver campus, by Skype and by telephone.
The clinic accepts referrals from health care professionals, as well as from caregivers and self-referrals from patients. More information about the Clinic is available at: http://clinic.pharmacy.ubc.ca/ or by calling 604-827-2584.
